Production Contract:
Required to get free community use of equipment to produce programs.
General Release:
It is recommended that anyone speaking on camera sign one of these.
Multiple Release:
If you have lots of actors or speakers on camera.
Event of the Week Reservation:
For organizational members only to arrange simple video coverage.

Live Programming Forms

BeLive! Agreement:
Application for a live 27-minute show.
CCR Program Request:
Application for a regular 2 hour, audio only program.
Bulletin Board:
Submit this to publicize your event on our channels.

Cablecast Request Forms:

Cablecast Agreement:
Application to schedule preproduced programs on our cable channels.
Bandwidth Local Music Project:
Use this to join the CCTV database of local musicians!

Professional Services:

Rental Rate Card:
Our recently updated rental rates for all of our equipment.
Duplication Rates:
Rates for copying tapes, MiniDV, SVHS, VHS, and one pass DVD.

Sponsors and Friends:

Sponsorship Form:
Businesses and individuals may sponsor us at specific tiers.
Friends of CCTV Form:
Anyone may send a donation of any amount with this form.
